La Vista Community Center Summer Camp 2022 - Omaha Kids Can Run Club registration.

Open to kids in Grades K-6.

This session runs from Monday July 25th to Friday July 29th.

Cost is $49 for the 5-day Camp and includes all coaching, a T-shirt that we will tie-dye, and a medal that is earned on the last day of the session.

Registration is limited to the first 20 participants.

My child's participation in the program selected is voluntary. I understand that the selected activities may involve accidental injury and hereby voluntarily assume such risks. Knowing these risks, I want my child to participate in this activity. I (on behalf of my child) hereby assume the risk, and hereby waive, release, and discharge Omaha Kids Can Run, LLC, its officers, employees, activity instructors and assistants, and all officers and employees of the school or community center sites where said activity will take place, for any and all claims for damages for personal injuries, or claims for damages to property, which my child or my child's heirs, assigns, executors or administrators may have or which may accrue to my child's participation in this activity.

This permission is valid for the 2022 Summer Camp Session.

I consent Omaha Kids Can Run, LLC to use of any photographs or video recording that are taken of my child while participating in the activity for use in Omaha Kids Can Run, LLC brochures and program materials that are distributed both as printed document and on the internet. No payment will be made for use of these photographs and/or videos. Your child's name would never be used in connection with these images.

REFUND POLICY:*

Refunds will not be issued for cancellations or withdrawals

***Exceptions to this policy apply only to COVID-19 circumstances:
-If the directed health measures change before the beginning of the session and we are unable to meet in person, all participants will have the option to receive a refund (minus the processing fee).
-If the directed health measures change at any time during the 5-day camp session, the participants will have the option of a prorated refund (minus the processing fee) OR to continue participating in virtual classes to complete the camp.

**The processing fee is non-refundable.

COVID-19 Precautions*

I understand that my child will not be able to participate any day that he/she has a fever. I also agree to keep my child home if he/she has been exposed to COVID-19 or is exhibiting any symptoms.

MAKE-UP CLASSES & TRANSFER POLICY:*

Make-up classes & transfers will not be accommodated during the Summer Camp Session.

LATE POLICY:*

Child will need to be picked up at the end of the scheduled class. There is a late pick-up charge of $5.00 for every 5 min of tardiness.

CLASS CONDUCT:*

A child whose behavior becomes unmanageable even after verbal and written warnings will be released without refund of program fees.

Additional Authorized Individuals for Pick Up:

* Children will ONLY be dismissed to authorized individuals, 16 years of age or older, with proof of identity. In cases when someone else will pick up your child, please provide written notice in advance.
